Both forms are Adobe pdf files. Print out the Parent Consent Form and have your parent/guardian fill it out - turn it in to your English teacher on Monday, March 23.
The Information Form contains question that you need to ask your job shadowing sponsor - contact your job shadowing sponsor by Friday, March 20th. Type the answers in the blue box and print out the form. The form is due on Monday.
